WOONSOCKET, R.I. - Accordant, a CVS Health company, announced Wednesday that it has received Case Management Accreditation from NCQA.

"At Accordant, we provide individualized support to help keep members on track with their care, which helps avoid unnecessary medical costs for both members and clients while improving overall health outcomes."
As part of the Case Management program, Accordant manages care transitions and identifies and prevents potential problems to help members avoid emergency room visits and extended hospital stays. The program is supported by published guidelines and consensus statements as well as standards of medical practice. NCQA evaluates health care companies against 10 standards of case management developed with input from experts from the field, including researchers and state and federal regulators.
