DURHAM, N.C. — Kmart pharmacies are implementing Pharmacy Quality Solutions' EQuIPP information-management platform to aid in the tracking of quality performance metrics and to enhance patient-centered care and clinical outcomes.

Through EQuIPP, Kmart pharmacies will be able to track their performance on numerous quality measures that are important to health plans. This includes triple-weighted quality measures for Medicare Part D plans, as well as medication adherence rates included in the new Quality Rating System for health insurance exchanges. Pharmacists can also identify patients with quality-related care gaps to support efforts to optimize the quality of services for their patients.
